ETUDES Live Shell Request This form should be submitted to the Distance Education office at least two months before the semester begins. Due to changes, forms for the semester can be requested up to the mid-semester.

Online Teaching Certificate Course Waiver The Course Waiver form is completed by the instructor for approval of acceptance to teach distance education courses at ECC.  This is completed only if an instructor has completed training or taught online courses at another institution.
